MSW Geriatric Care Manager
Fulltime MSW to work with clients throughout King and Snohomish Counties.  

Duties include:
  • Conduct comprehensive Assessments and development of Care Plans
  • Counsel clients and their families on issues of aging, life transitions and other care systems
  • Manage a caseload of vulnerable adults receiving Geriatric Care Management and In-Home Care 
  • Provide Crisis Intervention as needed
  • Develop and maintain Professional Networks on behalf of CareForce
  • Participate in rotating after hours On-Call with team of clinical professionals

Requirements include:
  • A Masters in Social Work from an Accredited School of Social Work
  • Mental Health Professional and/or Geriatric Mental Health credentials preferred
  • Two years Home Health or Acute Care experience - Care Management preferred
  • Ability to read and write English proficiently
  • Valid Washington State drivers license and insurance
  • Safe operating automobile and interest in driving within the community for daily assignments

RN Geriatric Care Manager
Fulltime RN to work with clients throughout King and Snohomish Counties, 50% of the work week will be based in our Lynnwood office and 50% will be out of our Tukwila office.  

Duties include:

  • Conduct comprehensive Client/Family Assessments
  • Development of Care Plans and monitor condition and progress
  • Nurse Delegation, supervision and train caregivers 
  • Care Management, coordination of care and resources
  • Networking on behalf of CareForce
  • Participate in rotating after hours On-Call with team of clinical professionals


Requirements include:

  • A current and valid license to practice as a Registered Nurse in the State of WA, BSN preferred
  • Graduate credentials from an Accredited School of Nursing
  • Two years Home Health or Acute Care experience - Care Management preferred
  • Ability to read and write English proficiently, communicate with clients, families and personnel
  • Demonstrated computer skills to function comfortably in an office environment
  • Valid Washington State drivers license and insurance
  • Safe operating automobile and interest in driving within the community for daily assignments
