CareForce offers professional Geriatric Care Management services for seniors living in Seattle, Bellevue, Everett and the surrounding areas throughout King County and Snohomish County.

Our Registered Nurses and Masters Level Social Workers are available to help you manage the care of your loved one wherever they reside in King or Snohomish County.  Whether you live thousands of miles away from the Puget Sound, or as close as Seattle, Bellevue, Edmonds or Everett, our professional Geriatric Care Managers are available for assessment, consultation and assistance in managing the care or challenges of your spouse, partner or close family member.

Care Managers can assist if you simply need a consultation on the care needs of your family member, wish to have respite from your responsibilities for a vacation or difficult period of time, or require full professional management of the care needs of your loved one.  We can serve as a ‘coach’ as you learn to better manage the care yourself, serve as an advocate or manage any or all aspects of the care needs of your loved one.  We will keep you informed and involved as much as you wish and provide you with peace of mind whether you live in King or Snohomish County or live large distances from the greater Seattle area.

At CareForce you can begin your journey with a care manager and adjust your reliance as your situation changes.

Our Care Management and Assessment Services include:

  • Complete assessment of physical, psychosocial, cognitive, and emotional needs with recommendations for community service.
  • Assist with enrollment and referral to appropriate community resources, including medical, dental, social, health care prevention and maintenance services. Provide support, coordinate appointments, and arrange transportation as needed.
  • Provide coaching and communication with family members or legal representatives to develop care plans and to assist as needed the care management.
  • Provide consistent communication and facilitation with health care providers and family members about health care issues.
  • Assist families and clients to develop a strategy for aging in place or moving to a different level of care.
  • Provide consultation on client needs for health care or social service assistance.
  • Coordinate with private caregivers and other service providers. Our care managers  will not hire, fire, or supervise private caregivers, but they will communicate with them and provide feedback to families or legal representatives regarding their observations.
  • Serve as liaison with home care services when extra assistance is needed. We DO NOT refer our clients  exclusively to our own home care services. Our Care Managers receive no incentives or pressure to recommend CareForce services. They are ethically responsible to act in the best interest of their clients at all times. Our Care Managers will recommend only those services, professionals, and organizations that they firmly believe best accomplish the needs of the client and established plan of care.  That is consistent with the CareForce Mission, values, and practices; it is how we do business and how we have earned the trust of so many families and the finest eldercare professionals.
